Admission is granted to those who qualify with the definite understanding that the student and his/her parents or guardians agree to comply with the scholastic standards of the Sisters of Mary Immaculate School. The authorities of the school claim the right to treat violations in accordance with their system of discipline and guidance based on Christian principles.
Submit the following documents at the Registrar’s Office (Window 1)
a. Original and photocopy of Baptismal and PSA Birth Certificates
b. Original Report Card with Principal’s signature (Form 138)
c. Certificate of Good Moral Character (signed by the Principal or Guidance Counselor)
d. Diploma or Certificate of Completion in Kindergarten Level/Grade 10 level (for in-coming Grade 1 /Senior High School- Grade 11)
e. Voucher certificate for Senior High School (Applicants from Private School)
f. Medical Certificate from Family Physician/Pediatrician stating previous and present illnesses, allergies, family history of illnesses and other precautions regarding the student. (for Pre-school especially)
g. Entrance Examination Fee (Php. 250 for Pre-school; Php. 350 for Grade I to High School to be paid at the Cashier)
1. Verify the following at the Registrar’s office (Window 1)
New pupils/students
a. Principal or Guidance Counselor’s recommendation after interview
b. Result of the Entrance Examination
Old pupils/students
a. Result of Interview of students under Probationary Status
b. Released Form 138 (Report Card)
2. Secure and fill out a Registration Form from the Registrar
3. Return the duly filled out Registration form at the Registrar’s Office for enlistment
4. Proceed to Window 2 for Assessment
5. Proceed to Window 3 (Cashier) for payment of fees
6. Proceed to the releasing of book area and present the Receipt to the In-charge to claim the books, student’s handbook, and SMIS (for Pre-school to Grade school)
7. Proceed to Window 4 (Bookstore) for school uniforms.
Note: Penalty of Php 300 for Late Registration will be charged for payment made on the first day of classes and onwards.
TEST AND INTERVIEW RESULTS
The Test result will be released on dates scheduled by the Admission Office. Parents and student are required to come for the result and for an interview with the Principal or Guidance Counselor.
